Interprofessional Primary Care Team

About this Program

The Interprofessional Primary Care Team at Scarborough Centre for Healthy Communities offers coordinated care through a team of health professionals. We aim to improve access and support for clients with complex needs. Referrals can come from a doctor, nurse practitioner, or be self-initiated for most services.

Any person needing inter-professional services in the Scarborough community can be referred. Clients can expect to enter a community of care-focused providers who will work together to help achieve their health and wellness goals.

The IPPC team consists of Nurse Practitioners, Physiotherapists, Social Workers, Registered Dieticians, Registered Nurses, Mental Health Case Workers, Harm Reduction Workers, Chiropodist, Foot Care Nurse, and Medical Secretaries.

The IPPC Team is pleased to offer culturally safe services and interpretation upon request.

  • Primary care for children & adults
  • Women’s Health
  • LGBT+ Care, HIV Pre-Exposure Prophylaxis (PrEP) & medical transition care
  • Chronic Disease Management
  • Harm Reduction – clean supplies and support for problematic substance use
  • Nutrition
  • Clinical counselling
  • Foot Care
  • Physiotherapy
  • Community workshops, groups and presentations
  • Smoking Cessation

Eligibility

  • The IPPC team welcomes referrals from family doctors and nurse practitioners. Referrals can be faxed to: (416) 410-7072.
  • Self-referrals are accepted for most programs; please contact Central Intake at 416-847-4134 or intake@schcontario.thedev.ca
  • A doctor’s referral is required for physiotherapy services.
  • We are located in Scarborough and serve individuals and families living in Scarborough and the surrounding areas.
  • We are unable to provide services to individuals in Canada on a visitor’s, student’s, or super visa.
